Abstract Details

Purpose:

To describe the clinical case of ligneous conjunctivitis and difficulty of current treatment, because there is no definitive treatment. Being associated with deficit of plasminogen, the use of topical heparin may improve the patient's condition.

Setting:

Virgen Macarena University Hospital. Seville, Spain

Methods:

Clinical case. Female 46 years after lower eyelid retraction surgery retraction develops membranous conjunctivitis, which stay at 3 months. It´s confirmed plasminogen deficiency. (values: 22% and 25% ) Treatment: Prednisone 1 mg / kg body weight and day and azathioprine 50 mg / 8h and topical: Cyclosporine 2%, Prednisolone, Enhanced Heparin (2600 UI/mL) and intensive autologous serum. Left eye requires removal surgery of the membranes with synthetic dural reconstruction.

Results:

Membranes of Right Eye have almost totally disappeared with treatment. The membranes of left eye are still active.

Conclusions:

Ligneous conjunctivitis associated to plasminogen deficiency currently has no effective treatment. The use of topical heparin may be acceptable to control and eradicate membranes. No complications from its use. Surgery should only be performed when the membranes endanger vision, because after surgery is reactivated the inflammatory process.
