Abstract Details

Purpose:

To evaluate and decode causes of suboptimal visual outcome in patients undergoing refractive surgery.

Setting:

Refractive surgery department in Narayana nethralaya super specialty eye hospital.

Methods:

Patients who complained of suboptimal quality of vision after refractive surgery between February 2013 and December 2014 were enrolled in the study. Patients with refractive error of more than +/- 1D following refractive surgery were excluded. All patients underwent dry eye evaluation, aberrometry & epithelial profiling. A select group of patients underwent additional tests like multifocal ERG and field examination when needed.

Results:

115 patients were included in the study. Dry eye was found to be the most common cause of suboptimal visual outcome (47.82%). Epithelial irregularities were found in 10.43% of patients. High corneal and lenticular aberrations were found in 21.73% and 15.65% of patients respectively. Miscellaneous causes of suboptimal vision which included retinal diseases like occult macular dystrophy were found in 4.34% of patients.

Conclusions:

Dry eye was found to be the most common cause of suboptimal visual outcome in our study. Refractive surgery surprises can involve the most unexpected diagnosis. Conventional tests may not reveal the cause in many cases and these patients may require in depth optics evaluation and specialised tests as the case demands.
