Purpose:

The subluxation of phakic, iris-fixated intraocular lenses (IOLs) is a rare complication. We report about the findings in spontaneously subluxated Artisan/Verisyse and Artiflex/Veriflex IOLs (Ophtec/AMO, Netherlands/USA) using light and scanning electron microscopy.

Methods:

Explanted Artisan/Verisyse and Artiflex/Veriflex IOLs were analysed in our laboratory using light and scanning electron microscopy. The IOLs had been explanted because of spontaneous subluxation leading to a decrease in visual acuity.

Results:

Light microscopy could detect decentred and twisted fixation arms. Scanning electron microscopy revealed micro-cracks in the material. There was no adequate closure of the fixation arms visible due to the defects in the material. Therefore, a reliable fixation in the iris stroma was not possible anymore. The defects in the material seem to be attributable to an unsuitable implantation technique.

Conclusions:

The subluxation of phakic, iris-fixated IOLs is a rare complication. The explantation of subluxated IOLs with deformed fixation arms is the only therapeutic option. A reenclavation of damaged IOLs would most likely result in repeating subluxations due to an unreliable fixation. The adherence to a correct implantation technique is of utmost importance to avoid spontaneous subluxations of phakic, iris-fixated IOLs.
