Abstract Details

Purpose:

To report two cases of bilateral oversized after Visian Implantable Collamer Lens (ICL, STAAR Surgical) surgery

Setting:

Clínica Oftalmológica TACIR (ES) Instituto de la Visión (PY)

Methods:

Two myopic woman with 25 Y.O. and 27 Y.O. , white-to- white measurements of 12.57 mm in the right eye and 12.54 mm in the left eye for the first patient, and white-to- white measurements of 11.80 mm in the right eye and 11.80 mm in the left eye for the second patient , the first one underwent simultaneous bilateral ICL implantation with VICM0 13.7 mm lenses and the second patient underwent in OU sequential ICL implantation with VICM 13.2 mm lenses. White to white measurements were taken with several methods (calipers,IOL Master, Pentacam)

Results:

Persistent, bilateral sub - acute angle closure developed despite multiple patent peripheral iridotomies and iridectomies in the VICM model however less pronounced IOP spikes was detected in the VICM0 model . Anterior segment tomography revealed profound vaulting of the ICLs and angle closure. All four ICLs were explanted.

Conclusions:

The correlation between white-to-white and sulcus- to-sulcus measurements were poor in these patient, resulting in extreme vaulting of the ICL and angle closure from a non-pupillary block mechanism. For non-pupillary block mechanisms, ICL extraction is necessary. The validity of the assumed correlation between white-to-white and internal posterior segment dimensions has been once againg put into question
