Can changes in higher order ocular aberrations one year after LASIK for a moderate to high myopic astigmatism be predicted in individual cases?

Abstract Details

Purpose:

To determine if alterations in the value of higher order ocular aberrations are related to pre-op values following astigmatic Laser in Situ Keratomileusis (LASIK) procedures using either one of two different platforms.

Setting:

University Eye Hospital, Svjetlost, Zagreb, Croatia

Methods:

Patients with moderate to high myopic astigmatism underwent unremarkable LASIK using either Wavelight Allegretto IQ 400Hz (group I,71 patients,126 eyes) or Schwind Amaris 750S (group II,64 patients,119 eyes) laser platforms. The higher order ocular aberrations (HOAs) were measured objectively using L 80 wave+TM, (Luneau SAS, Prunay-le-Gillon, France) pre- and 1 year postoperative in each case. The HOAs evaluated using this aberrometer were coma, trefoil, spherical aberration and the RMS value of the total aberrations for pupil sizes of 3mm and 5mm. The difference between pre-and post-op values (y) were compared with pre-op values (x) using standard linear regression.

Results:

Reporting the key significant findings (p=less than 0.05). For the 5mm pupil, linear regression revealed the following significant relationships between the changes of individual HOA values (y) and pre-op HOA values (x) Group I, n=126, Coma y = 0.654x-0.072(r=0.514) Trefoil, y = 0.677x+0.073 (r=-0.315) SA, y = 0.049-0.016x (r=-309) RMS, y = 2.955-5.561x (r=-0.285) Group II, n=119, Coma, y = 0.833x-0.111 (r=-0.503) Trefoil, y = 0.474x-0.035 (r=-0.355) SA, y = 0.471x+0.009 (r=-0.359) RMS, y = 0.888x-0.444 (r=-0.974)

Conclusions:

The gradient and abscissa values should be near zero if individual HOA values are unaffected by LASIK. Our results support an opposite view. The numerical indices in our linear expressions show that, both astigmatism correcting platforms tend to reduce the magnitude of the HOAs on an individual case by case basis. For both platforms, changes in HOA values can be reliably predicted using the pre-op values in individual cases for the 5mm pupil.
