Efficient and safe measures used in phacoemulsification to deal with various types of post-uveitic cataracts

Abstract Details

Purpose:

To asses and evaluate the efficiency and safety measures used in Phacoemulsification to deal various types of post uveitic cataracts.

Setting:

Khairabad Eye Hospital,Kanpur,India

Methods:

A total of 63 eyes of 54 patients with post uveitic cataract were operated with temporal clear corneal phacoemulsification in a period of two years between 1st January 2012 to 1st January 2014.The study included various types of post uveitic cataracts namely occlusio pupillae,non dilating miotic pupil,festooned pupil,post trabeculectomy uveitic cataract and mature hard white uveitic cataracts. Standard preoperative regimen of topical as well as systemic steroids were instituted a week before surgical intervention. All cases were performed by a single surgeon under peribulbar anaesthesia using small pupil management devices.

Results:

Intraoperative complications were negligible with few incidents of anterior chamber instability (4.76%) , descemets stripping (1.59%),floppy iris (25.39%) ,uncontrolled rhexis formation (4.76%) and linear mark imprints at the pupillary margin due to iris retractors(17.46%). Post operative complications included day 1 postoperative corneal oedema (33.33%), fibrinous reaction(12.69%) 8 cases,increased intraocular pressure (11.11%) ,cystoid macular oedema (14.28%),pupillary capture (3.17%) 2 case,posterior capsular opacification (25.39%)16 cases.More complications were observed in hard uveitic cataract.A high percentage of (80.95%) recorded a best corrected visual acuity of 6/12 or better.

Conclusions:

Adequate exposure of pupillary area using pupil expanding devices and malyugin ring makes phacoemulsification safe and effective in these cases and helps overcome unavoidable complications.
