Epi-LASIK with intraoperative MMC for the correction of hyperopia and hyperopic astigmatism

Abstract Details

Purpose:

To evaluate visual and refractive outcomes after Epi-LASIK with prophylactic Mitomycin C (MMC) for the correction of hyperopia and hyperopic astigmatism.

Setting:

Refractive Laser Center Innsbruck

Methods:

In this retrospective study 28 hyperopic virgin eyes from 15 consecutive patients were included. Mean age of patients was 45.12 years (range 26-61). An epikeratome (Gebauer, Germany) was used to create the epithelial flap which was discarded after excimer laser ablation (Wavelight Allegretto Wave Eye-Q 400Hz, Germany). In astigmatic eyes, limbal marks denoted the horizontal axis. After photoablation, a sponge soaked in 0.02% MMC solution was applied for 20 seconds, followed by extensive irrigation with balanced salt solution. Visual acuity, possible corneal haze formation and refractive stability were evaluated early postoperatively and 6 to 12 months after surgery.

Results:

Preoperative hyperopia ranged from +1.25 to +4.0 D (mean +2,528 D) with up to - 4.0 D of astigmatism (mean -0.978 D). Early postoperative mean refractive error was -0.305 D (range -1.75 to +1.25 D) which stabilized at a mean value of +0,920 D late postoperatively. Mean cylindrical error was -0.529 D (range 0.00 to -1.25 D) early postoperatively and further improved to -0.402 D at 12 months. Early postoperative UCVA was 0.748 and improved to 0.924 at 12 months after surgery. Only trace haze was observed in 2 patients and there was no loss of BCVA in all patients.

Conclusions:

Until now, only few data are available in the literature regarding the efficacy of MMC in hyperopic corrections. In the present study we could show that Epi-LASIK with intraoperative 0.02% MMC was efficacious to inhibit corneal haze formation and regression in patients with hyperopia up to +4.0 diopters. Interestingly, outstanding results could be obtained in patients with hyperopic astigmatism with stable results up to 1 year after surgery. MMC seems to be a useful adjunct for Epi-LASIK in hyperopia and hyperopic astigmatism.
