Abstract Details

Purpose:

To evaluate the predictability of direct smear in the diagnosis of post LASIK infection using gram staining

Setting:

Research Institute of Ophthalmology, Cairo, Egypt

Methods:

Two cases with whitish corneal lesions and infiltration presented one week after LASIK were managed with empirical treatment and close follow up. Came later with worsening in the clinical signs (edema of the flap and increase in infiltration with focal satellite whitish lesions) The flap was relifted, washing under the flap with antibacterial and antifungal after taking swabs for direct smear, gram staining, culture and sensitivity.

Results:

Immediate improvement in both cases was observed. The specimens were positive for fungus in the direct smear, however the results for culture and sensitivity were false negative.

Conclusions:

Gram stain was found to be reliable for making decisions in the treatment of bacterial and fungal keratitis, thereby enabling the clinician to start appropriate treatment immediately.
