Purpose:
To evaluate visual outcomes, postoperative change in vault and to assess potential impact on crystalline lens density in eyes after phakic posterior chamber Implantable Collamer Lens (ICL, STAAR Surgical AG) implantation to correct moderate and high hyperopia.
Setting:
European Eye Clinic Lexum, Czech Republic
Methods:
Our study comprised 31 eyes (17 patients) after implantation of a phakic posterior chamber ICL (model V3) to correct moderate and high hyperopia. Postoperative examinations were scheduled at 1 week, 1 month and 1 year. Analysed parameters were preoperative and postoperative refraction and logMAR uncorrected (UDVA) and corrected (CDVA) distance visual acuity. Postoperative central vault and peak value in crystalline lens densitogram were measured using Scheimpflug imaging (Pentacam, Oculus Optikgeräte GmbH).
Results:
Mean age of patients was 28±5 years (18 to 36 years). Mean UDVA improved from preoperative 0.70±0.34 to 0.13±0.16 logMAR one year after the surgery. Mean spherical equivalent decreased from 6.98±1.51 D to postoperative -0.34±0.46 D. Mean vault decreased from 371±245 µm at 1 month to 323±230 µm at 1 year (p=0.007). Densitometry changed from preoperative 10.11±1.96 to 10.98±1.67 at 1 month and to 9.97±1.19 at 1 year. Densitometry at 1 month was higher (p=0.003), while the difference between preoperative and 1-year densitometry was not significant (p=0.586). No serious complications occurred.
Conclusions:
Implantation of a phakic posterior chamber ICL in hyperopic eyes led to a significant improvement of uncorrected visual acuity and refraction. Despite an increase at 1-month visit, 1-year crystalline lens densitometry did not differ significantly from preoperative values, suggesting no or little impact of surgery on crystalline lens density. However, further monitoring of patients for potential cataract formation is needed due to the significant decrease of vault.
