Abstract Details
Purpose:
To observe femtosecond laser–assisted capsulotomy in eyes with intumescent white cataract, including capsulorhexis size, integrity, and intraoperative and postoperative complications, and postoperative intraocular lens in neutral,and evaluate the feasibility and safety of femtosecond laser–assisted capsulotomy in eyes with intumescent white cataract.
Setting:
This is a prospective clinical trial.
Methods:
After femtosecond laser–assisted capsulotomy (LenSx system), phacoemulsification was performed using twist ultrasound energy and had hydrophobic acrylic intraocular lens SN60WF IOL(Alcon) implantation. The lenticular capsule disk was stained intraoperatively with indole green and pulled out using a microsurgical forceps for further histopathologic evaluation, observe the average deviation of the actual diameter and the targets set diameter, and the cutting quality of histopathological examination. Observe the complications of intraoperative and postoperative, and use the Pentacam section measurement system to detect IOL eccentric and tilt after 1 month, 3 months, 6 months.
Results:
All twenty-five eyes with intumescent white cataract were successfully completed femtosecond laser–assisted capsulotomy under OCT monitoring. In all cases, the intraocular lens was centered and the implantation was uneventful. None of the pouch distorted and suspensory ligament rupture, and complications directly related to the laser. Radial anterior tears occurred in 1 eyes, an adherent tongue-like capsule adhesion in 3 eyes, and an incomplete capsulotomy button in1 eyes. But the surgery was still successfully completed, posterior capsular rupture and vitreous prolapse did not occur, and 6 months of follow-up after surgery did not detecte obvious IOL eccentric or tilted. The mean deviation from the target diameter of the extracted capsule disks was 60 μm ± 44 (SD).
Conclusions:
Femtosecond laser assisted capsulorhexis is accurate and reproducible, femtosecond laser–assisted capsulotomy in eyes with intumescent white cataract is feasible and safe.It helps to avoid complications duo to capsule pressure increase.The safety of surgery can be improved in high risk patients. Femtosecond laser assisted capsulorhexis is accurate circle and center avoids intraocular lens, eccentricity and tilt caused by capsular bag contraction after implantation, so as to ensure the long-term stable postoperative visual quality. So in all intumescent white cataract surgery using femtosecond laser assisted capsulorhexis is safe and feasible in technology.
