Purpose:
To measure the decentration as angle kappa (K) with ray tracing aberrometry (iTrace)and its effect on the Total Post Operative Spherical Aberrations (TPOSAs) as well as visual function and satisfaction levels measured using Visual Function 14 Quality Of Life Questionnaire ( VF-14) in patients implanted with Bilateral Diffractive Aspheric Multifocal Intraocular Lenses (MFIOLS).
Setting:
Tertiary super specialty eye care hospital, Bangalore, Karnataka, India
Methods:
Patients opting for bilateral diffractive aspheric MFIOL implantation post cataract surgery , were included in the study. Six weeks postop they were tested for Corrected Distance Visual Acuity (CDVA) and residual refraction. Decentration, using Angle Kappa, and TPOSAs were measured using the iTrace . They were also asked to fill the VF-14-QOL questionnaire at the same visit. Kolmogorov-Smirnoff test was used to test normalcy of data. As the data was found to be non-normative Spearman's co efficient of correlation was used to test for significance and values of <.05 was taken as positive.
Results:
54 eyes of 27 patients were included, mean age 62.03+/-9.91 years, CDVA was 20/20 amd N6 in all the patients Mean Residual Spherical Equivalents in MFIOL were -0.17 D in the right and 0.075 D left eyes respectively. Average TPOSAs was 0.0007 µ +/- 0.036 µ, Average VF 14 Score was 94.56+/-6.98, Average value of Angle kappa (mm) was 0.24 mm+/-0.22 mm. On analysis we did not find any positive correlation between Angle Kappa and TPOSAs (p= .137) or VF-14 scores (p=.116). We found a significant negative correlation between TPOSAs and VF-14 scores (p=.026)
Conclusions:
Increased spherical aberrations in our set of patients correlated well with decrease in satisfaction scores. However Decentration of MFIOLS away from the visual axis did not lead to increased Spherical aberration, or a reduction in satisfaction scores. In our study using the current generation of aspheric diffractive multifocal lenses, decentraion within our data range did not affect spherical aberrations or patient satisfaction levels significantly.
