Abstract Details

Purpose:

Evaluation of the most frequent long-term, post-operative complications after congenital cataract surgery

Setting:

Ophthalmology clinic of the 2nd Medical Faculty, Charles University in Prague, and the Faculty Hospital Motol

Methods:

A retrospective study evaluating records of paediatric patients that were operated on up to the age of 4 months between 2005 - 2013. The group of 152 eyes was divided into 77 artephakic eyes (with primarily implanted intraocular lens) and 75 aphakic eyes. In these patients we observed the incidence of the three most frequently occurring long-term post-operative complications that were treated surgically: secondary glaucoma, after-cataract and cyclitic membrane.

Results:

Secondary glaucoma occurred in 44% of eyes, in 19% it was treated by cyclodestructive or filtration surgery. After-cataract was diagnosed in 45.9% of eyes, and in most cases 45% it was treated using pars plana aspiration. Cyclitic membrane occurred in 23.6% of eyes, and was treated surgically - in 23.0% by discision of the membrane. In the case of primarily indicated congenital cataract surgery we observed an incidence of secondary glaucoma higher in the group of aphakic eyes (25.6%) than in artephakic eyes (18.4%). The incidence of after-cataract was, on the contrary, higher in the group of artephakic eyes (38.1%) than in aphakic eyes (7.8%), just as cyclitic membranes were more frequent in artephakic eyes (21.0%) than aphakic eyes (2.6%).

Conclusions:

The incidence of secondary glaucoma was higher in the group of aphakic eyes. The incidence of after-cataract and cyclitis membrane was singnificantly higher in the group of artephakic eyes. The percentage of these complications described above is significant, therefore, in post-operative care, regular observation and early detection and treatment of these complications is very important.
