Purpose:
To evaluate the safety and efficacy of topography guided laser subepithelial keratomileusis (LASEK) in selected post-radial keratotomy (RK) eyes with hyperopic shift and/or astigmatism.

Ozel Karsiyaka Goz Hastanesi
Methods:
The study comprised 22 eyes of 16 patients who had uneventful RK and subsquently developed a hyperopic shift. LASEK was performed using Schwind Esiris excimer laser system. All RK eyes had 10 or 12 radial incisions, were more than 1 year post-RK and had no epithelial inclusion cysts and corneal disease. Main outcome measures were uncorrected (UDVA) and corrected (CDVA) distance visual acuities, spherical equivalent (SE), and haze. All eyes were followed at least 1 year post-op (range 12-18 months).
Results:
The mean SE was corrected from +3.12±1.02 diopters (D) to +0.28±0.58 D after LASEK (P less than 0.001). The mean astigmatism was corrected from -1.78±1.22 D to -0.56±0.66 D (P less than 0.001). Preoperative CDVA and UDVA were 0.26±0.12 and 0.81±0.16 logMAR, respectively. At the end of 1 year follow up, CDVA and UDVA were 0.11±1.12 and 0.28±0.15 logMAR, respectively (P less than 0.001 for both). At 1 year, UDVA was 20/25 or better in 47.4% of eyes and 20/40 or better in 77.8%. Two eyes developed central haze not effecting the visual acuity.
Conclusions:
Topography guided LASEK is safe and effective for correcting hyperopic shifts in properly selected RK patients with a low incidence of visually significant corneal haze.
