Purpose:
Evaluation of the efficacy, predictability and safety of wavefront-guided refractive laser treatments (Lasik and Photrefractive keratectomy (PRK)) using a new-generation Hartmann-Shack aberrometer.
Setting:
University hospital of Brest, Department of Ophthalmology, Brest, France
Methods:
445 eyes of 241 patients were treated with either PRK or Lasik (flap of 100 microns using the IFS femtosecond laser and ablation carried by STAR S4 IR excimer laser (Abbott Medical Optics, Santa Ana, California) for myopia or hyperopia (≤ 8D) and / or astigmatism (≤ 5D). Patients were included in a prospective study between December 2012 and August 2014. The customized ablation profile was guided by the new aberrometer iDesign (Abbott Medical Optics, Santa Ana, California).
Results:
286 eyes underwent LASIK and 159 a PRK.. 53% of eyes needed a physician adjustment in addition to the recommanded nomogram.
In the myopia group, 3 months postoperatively, Spherical equivalent was within ±0.50D in 100% of patients that underwent a PRK and in 97.9% of those treated by LASIK. Best corrected visual acuity was equal or better than preoperatively in 95% of eyes. Stability was noted from 6 months.
Near 90% of patients were satisfied and would recommend it to their relatives .
Iris Recognition was obtained in 98% of cases allowing for compensation of cyclotorsion and pupil centroid shift.
Conclusions:
The customized refractive laser treatments guided by the new-generation aberrometer is safe and predictable for both surface ablation and lasik in myopia and hyperopia and allow to obtain better refractive results and quality of vision with a reduction of functional symptoms and hight rates of of patient satisfaction. Since the introduction of iDesign retreatmentssignificantly decreased in our center (<1%). It is our preferred laser treatment over conventional ablation.
