Purpose:
To assess clinical results of transepithelial topography-guided PRK performed with Microscan Visum excimer laser for irregular corneal astigmatism correction
Setting:
S. Fyodorov Eye Microsurgery Federal State Institution
Methods:
Prospective, non-comparative analysis of case series. Study group included 25 eyes of 25 patients with secondary irregular astigmatism after radial keratotomy (RK), keratitis, decentrated ablation and penetrating keratoplasty (PK). All included patients had before treatment less or equal 20/60. An ablation profile based on corneal topography data and manifest refraction data was created using KeraScan software. Surface ablation was performed using Microscan Visum eximer laser (Optosystems, Troitsk, Russia). Postoperative assessments (BCVA, UCVA, contrast sensitivity, corneal topography) were performed at 1 week and 1, 3, 6 and 12 months
Results:
Mean attempted spherical equivalent (SE) refraction was –5.8±0.79 D. Mean attempted cylinder correction was -6.3±1.02. Median preoperative BCVA was 20/55 and median preoperative UCVA was 20/100. Efficacy: 84% of eyes achieved UCVA 20/40 and more. 64% achieved BCVA 20/32 and more. Safety: No eyes have lost lines of BCVA. 24% of eyes gained 4 lines of BCVA, 68% gained 2 lines or more. Accuracy: mean deviation from the intended SE was -0.43±0.68 D. 72% of eyes were within 1.5D of intended SE. There was a statistically significant improvement in contrast sensitivity and surface irregularity index
Conclusions:
Topography-guided PRK is effective for improving visual acuity and quality of vision in patients with irregular corneal astigmatism.
