Purpose:
Comparing refractive outcomes and changes in corneal curvature between SMILE and LASIK in high myopic patients
Setting:
Vissum Corporation, Alicante, Spain
Methods:
A retrospective study of a series of cases, comparing the refractive out comes of 2 surgical techniques using 3 platforms, group A underwent Small Incision Lenticular Extraction (SMILE) using the VisuMax by Zeiss in comparison with Laser-Assisted in situ Keratomileusis (LASIK) in 2 groups; group B using AMARIS by Schwind and group C using MEL 80 by Zeiss. Mathematical model was used to calculate the tensile property between matched groups of SMILE and LASIK, based on the differences between targeted and obtained corneal curvature, as a theoretical measure of the biomechanical prosperity of the cornea after surgery.
Results:
Group A of 96 eyes of 75 SMILE patients, mean age of 33.2 years with spherical equivalent (SE) of -7.71 D (-5.55: -10.0), group B 48 eyes of 27 LASIK patients with AMARIS, mean 34 years with SE of -7.85 D (-6.00:-11.13), group C LASIK with MEL 80, mean 35.6 years with SE -7.87 D (-5.53:-11.38). 76% SMILE had UDVA 20/20 or more with, 81% and 78% for groups B & C respectively. 93% of SMILE did not loss any lines, 98% for B & C. 78% of SMILE within 0.50D of target, 75% and 64% for B & C.
Conclusions:
All three groups showed comparable results correcting high myopic patients; both excimer laser platforms showed slightly higher efficacy and safety than SMILE, on the other hand, the SMILE had the highest accuracy in all groups. Small Incision Lenticular Extraction is an safe and accurate modality for correcting high myopic refractive errors.
