Purpose:
To assess the factors affecting early postoperative corneal edema (EPCE) after phacoemulsification in patients without any corneal pathology, and also evaluate EPCE in patients with thick central corneal thickness (CCT) in comparison with patients with normal range of CCT.
Setting:
Retrospective observational study from the single tertiary hospital
Methods:
We retrospectively reviewed the medical records of 66 eyes of 44 patients who underwent phacoemulsification without evident corneal endothelial pathology, and analyzed endothelial cell density (ECD) and CCT at preoperatively and 1 month postoperatively. We evaluated the effect of patients factors (age, CCT, anterior chamber depth (ACD), ECD) and surgical factor (cumulative dissipated energy, CDE) on EPCE. The subjects were divided into 2 subgroups: Group T (preoperative CCT ≥ 580 µm, 14 eyes); Group C (preoperative CCT < 580 µm, 44 eyes). Primary outcome measures were endothelial cell loss (ECL, %) and CCT change (%).
Results:
The CCT change was 1.6% in Group T and 2.9% in Group C (p=0.170). The ECL was -11.8% in Group T and -9.1% in Group C (p=0.701). The ECL and CCT change showed no significant correlations with age, CCT, ACD, and CDE (p>0.05). The CCT change had no correlation with preoperative ECD (p=0.798). The ECL and preoperative ECD has negative correlation (r=-0.602, p<0.001).
Conclusions:
Age, ACD, and CDE were not contributing factors for EPCE. The preoperative CCT doesn't affect EPCE after phacoemulsification in patients who don't have clinical endothelial diseases. The results suggest that ECL was lower in lower preoperative ECD patients with a negative correlation.
