Purpose:
To Investigate and highlight various corneal complications after manual small incision cataract surgery,to study risk factors leading to such complications,various preventive measures taken to reduce them,and final visual outcome following complications.
Setting:
Tertiary care teaching hospital in Rural part of South India.
Methods:
A Prospective Hospital based observational Study.In our study 100 eyes of 100 patients who underwent manual small incision cataract surgery with post chamber intraocular lens implantation,all performed by training young ophthalmologist at Narayana Medical college&Hospital Nellore between 14January 2014 14 january 2015.Corneal complications,their risk factors and final visual outcome were studied at1st day post operatively,end of 1st week,end of 1st month.
Results:
A total of 100 eyes of 100 patients who underwent manual small incision cataract surgery were studied for corneal complications 8 eyes (8%) devoloped striate keratopathy,5 eyes(5%)devoloped corneal edema >10 descements folds,1 eye(1%)devoloped epithelial downgrowth,1 eye (1%)devoloped Descements membrane detachment.Rest 82 eyes (82%) did not develop any corneal complications and none were converted to conventional ECCE.94 patients(94%)categerorized under Good visual outcome category,89 patients(89%) had Best corrected visual acuity of 6/9 or better at the end of 4th post operative week.
Conclusions:
Corneal complications like Striate keratopathy was transient and resolved after 1st postoperative week.Corneal edema with more than10 descements folds resolved gradually with useof topical medications and was clear at the end of 4th postoperative week.Descements membrane detachment and Epithelial downgrowths did not resolve completely however the final visual outcome was fairly good.Postoperative astigmatism of<1D was seen in 62% of eyes.Best corrected visual acuity at the end of 4th postoperative week was 6/9or better in 89%of eyes.Although corneal complication continue to reduce following innovations and modernisation in cataract surgery,right attitude,knowledge and timely management can reduce such complications by Young ophthalmologists.
