Purpose:
To report clinical and laboratory findings of 2 cases with bullous keratopathy induced by a foreign body in the anterior chamber after cataract surgery.
Setting:
Department of Ophthalmology, Kurume University Hospital, Kurume, Fukuoka, Japan.
Methods:
Case 1: A 73-year-old man underwent phacoemulsification (PEA) and intraocular lens (IOL) implantation with posterior capsule rupture in the left eye. Anterior vitrectomy was performed for IOL dislocation 3 months after cataract surgery, and IOL was placed in the sulcus. Bullous keratopathy developed 2 months after vitrectomy, and a foreign body was found in the anterior chamber. Case 2: An 87-year-old woman underwent uneventful PEA, IOL implantation, and goniosynechialysis in the left eye. She had a loss of vision 1 year after cataract surgery, and developed bullous keratopathy. An oily substance was detected in the anterior chamber.
Results:
A foreign body in the anterior chamber was removed and subjected to chemical analysis by Fourier transform infrared spectrophotometry. The foreign body was identified as silicone material consistent with the silicone sleeve of phacoemulsification handpiece in Case 1, and ofloxacin ophthalmic ointment instilled immediately after the conclusion of cataract surgery in Case 2.
Conclusions:
A foreign body in the anterior chamber can be a cause of bullous keratopathy after cataract surgery.
