Abstract Details

Purpose:

To evaluate exchanging implanted intraocular lenses (IOLs) causing visual or structural complications, for others improving vision and treating those complications.

Setting:

Research Institute of Ophthalmology (RIO), Cairo, EGYPT.

Methods:

This study included 18 eyes of 14 patients. Ages ranged from 8 to 70 years (mean 42.2). Four were bilateral and 10 unilateral cases. Those included: misplaced phakic IOLs (P IOLs) , miscalculated biometry, opacified hydrophylic acrylic IOLs, traumatic and spontaneous IOL displacement and bullous keratopathy. IOL exchange was done with or without cataract extraction. IOL exchange was associated with cataract extraction and posterior chamber IOL implantation in three cases with PIOLs . Two eyes had misplaced P IOLs causing complicated cataract and the third was displaced causing traumatic cataract.

Results:

IOL exchange caused vision improvement ranging from hand movement (HM) preoperative to 6/6 postoperative. Bullous keratopathy resolved in 66.6 % of cases while iridocyclitis, secondary glaucoma and diplopia resolved in 100 % of cases. Significant visual improvement and resolution of complications were recorded.

Conclusions:

IOL exchange was mandatory to correct vision and treat complications. Timing of surgery and pre and postoperative treatment was important in improving results.
