Purpose:
IOL Exchange as we know requires the use of Micrograspers, Microscissors, Snares etc. Here we describe a technique to perform IOL exchange with routine instrumentation
Setting:

Methods:
Once IOL is free bring IOL in such a position that the optic haptic junction is near the main wound.Fix IOL with a chopper at distal optic junction and with a Vannas start cutting the IOL at the proximal optic haptic junction.Always take small bites with Vannas to avoid IOL tilt.Always make sure that the cut is in a single track.While cutting IOL give constant counter pressure with chopper.After cutting the IOL grasp one haptic which is near the primary incision with McPhersons and explant first half.Remove the other part by holding the other haptic.
Results:
After explantation of that Hydrophilic Toric acrylic lens a single piece Acrysof IQ Toric IOL was implanted in the bag
Conclusions:
With good surgical techniques a successful IOL exchange can be performed without complications and with routinely used instruments.
