Abstract Details

Purpose:

Laser Blended Vision for Presbyopia by Carl Zeiss Meditec is a wavefront-guided excimer laser treatment that has several mechanisms to increase near vision keeping a good vision for distance as well: micro-monovision, depth of field, spherical aberration, vertex centration, retinal image processing, neural summation, blur adaptation and neural suppression. This makes it an excellent option to consider for presbyopic patients compared to multifocal intraocular lens implantation that produces a decrease of contrast sensitivity and can even have major complications. We studied the results of 230 consecutive patients that underwent Laser Blended Vision treatment in our clinic.

Setting:

Retrospective study of 230 consecutive patients (445 eyes) that underwent LASIK with the wavefront-guided Laser Blended Vision programme for presbyopia by Carl Zeiss Meditec during the last 4 years in our clinic. Optima Laser Eye Clinic, Valencia, Spain.

Methods:

The programme has a non-linear aspheric ablation profile that increases the spherical aberration. A slight myopia of -1.5 spheres in the non-dominant eye is programmed. 100 male and 129 female patients between 42 and 71 years old. Only 3.91% were between 42 and 44 years; 31.74%,between 45 and 49; 64.35% were 50 years or more. The follow-up was between 1 to 41 months. 3.59% of eyes were emmetropic, 25.17% were myopic (myopic astigmatic) and 71.24% were hyperopic (hyperopic astigmatic). 15 patients underwent the surgery in the non-dominant eye only. The efficacy, security, predictability and patient's satisfaction were analysed.

Results:

97.09% of the hyperopic or emmetropic patients had a pre-op near vision between Jäger4-10. 98.84% of those patients have a post-op near vision between Jäger1-3; 83.14% have Jäger1. 72.41% of the myopic patients had a pre-op near vision Jäger1-3. 100% of those patients have a post-op near vision Jäger1-3; 96.55% have Jäger1. Post-op binocular visual acuity for the whole group: distance 20/20 or more: 96.09% had near vision Jäger1-3: 99.13% Jäger1 , 86.52% (efficacy).Security 99.78%. Predictability (0.5 dioptre) 93.03%. 95.22% were satisfied. No severe complications. Follow-up longer than 12 months (86 patients) and 95.35% are still satisfied.

Conclusions:

Laser Blended Vision for presbyopia has a high percentage of satisfaction for emmetropic, myopic, hyperopic and astigmatic patients with a fast adaptation and very good outcomes in visual acuity for distance and near. The results are as good for younger presbyopic patients as for older ones. Efficacy, security and predictability are high. Satisfaction is not always related to visual acuity, image processing is important to give a comfortable vision. The patient's satisfaction remains high in the long-term follow-up. Laser Blended Vision is an excellent option for presbyopic patients who prefer to avoid an intraocular procedure.
