Abstract Details

Purpose:

To compare defocus curve and contrast sensitivity function after implantation of +2.5D IOL in the dominant eye and +3D in the fellow eye with respect to a group implanted bilaterally with a trifocal IOL.

Setting:

Clinica Baviera, Madrid, Spain

Methods:

Single blinded prospective randomized controlled trial; phacoemulsiphication was performed by one single surgeon; minimal follow up was 3 months; patients were excluded if any intraoperative or postoperative adverse event or complication was noticed, and if final corrected distance visual acuity was not better than 0,2 LogMAR in any eye. Group 1 (n=11 patients, 22 eyes) implanted with mix and match bifocal AcrySof IQ ReStor +3/+2,5D was compared with group 2 (n=12 patients, 24 eyes) implanted bilaterally with Finevision trifocal (+1.75D, +3.5D) IOL. Monocular and binocular defocus curve and contrast sensitivity with the distance correction were compared with the Mann-Whitney test.

Results:

Monocular defocus curve comparaison between the group 1 eyes with +2,5D and the group 2 right eyes, showed a significant difference favoring trifocal IOL at -2D, -2.5D, -3D and -3.5D defocus addition (p<0.05). Monocular defocus curve comparaison between the group 1 eyes implanted with +3D and group 2 left eyes, showed a significant difference favoring trifocal IOL at -1D defocus addition (p<0.05). Binocular defocus curve comparaison showed a significant difference in favor of the trifocal IOL at -1D (p<0.01) and -3D (p=0.018 and above). No significant difference nor trend was observed between groups in the monocular and binocular contrast sensitivity.

Conclusions:

Both mix and match bifocal and trifocal implantations offer a continuum in the defocus curve from distance to near vision. Nevertheless the trifocal IOL displayed a superiority in both binocular and monocular outcomes without highlighting a significant difference in the contrast sensitivity.
