Abstract Details

Purpose:

To describe new technique of fishmouth compression suture in the management of corneal thermal burn due to phacoemulsification.

Setting:

Department of Cornea and External Disease, Medical Research Foundation, Chennai, India.

Methods:

Phaco thermal burn causes fishmouth opening of inner lip of phacotunnel. This wound does not close with routine closure of the outerlip instead it can lead to suture track leaks. Full thickness horizontal corneal suture one or both sides of the tunnel helps in achieving compression of the fishmouth opening of innerlip and tight closure of wound.

Results:

4 patients who had phaco thermal burn was managed with Fishmouth compresson sutures. Disperssive viscoelastic was used in all cases. Standard interrupted sutures to close the outer lips of the wound failed in all cases. Complete closure of the wound could be achieved with 1 compression suture in 1 case and with 2 compression sutures in 3 cases. Surgically induced astigmatism was less than 1D in all cases. Anterior segment OCT performed in the post operative period confirmed the wound closure.

Conclusions:

Fishmouth compression sutures are effective in the management of phaco thermal burn. One need to make sure there is free flow of irrigation fluid before starting phacoemulsification to avoid blocking of irrigation fluid by the viscodisperssive viscoelastic.
