Comparison of hydroimplantation, implantation under ophthalmic viscosurgical device(OVD) and autoSert motorized injector of hydrophobic acrylic foldable intraocular lens (IOL)

Abstract Details

Purpose:

To compare the intraoperative safety and ease of implantation of single piece hydrophobic foldable lens under irrigation,using OVD and autosert assisted implantation

Setting:

Divyadrishti Eye Centre,A superspeciality eye hospital at Patna,Bihar,India

Methods:

Prospective study of 750 cases of senile cataract who underwent phacoemulsification with 2.2 mm incision was conducted between January 2014 to December 2014.250 cases each were randomized to undergo standard implantation under OVD or implantation under irrigation or by autosert motorized injector.Single piece hydrophobic acrylic lens(SN 60 WF Alcon) using D cartridge was implanted in all.The main outcome measured were time and ease of implantation and IOL implantation related problems and complications.

Results:

Time required from implantation of the IOL to completion of hydration of wound was 60 +/-15 seconds,with OVD 2 minutes +/-15 seconds and with autosert 2.5 minutes +/-30 seconds.Implantation related problems and complications occurred in 13 cases,two and none in hydroimplantation,ovd and autosert implantation respectively.Failure of implantation occurred in six cases in hydoimplantation where IOL got trapped in the incision and could be inserted by forming the chamber by OVD.Flip IOL occurred in four cases in Hydro,one in OVD.Posterior capsule rupture in two case and hyphema in hydroimplantation due to sudden release of IOL in hydroimplantation technique.

Conclusions:

Though hydroimplantation technique takes less time for IOL implantation,it is associated increased intraoperative complications.Implantation using motorized injector though takes more time,it is the safest way of implantation of IOL.
