Abstract Details

Purpose:

To predict post-operative true lens position (TLP) from 3 dimensional algorithm derived intra-operative OCT (AD-iOCT) and to use this information to improve refractive outcomes.

Setting:

Veritas Eye Institute/ Cornea and Vision Science Foundation,Canada

Methods:

AD-iOCT predictions were compared to TLP as well as predictions from pre-operative biometry repeated twice, from two gold standard commonly used multivariate theoretical formulas, TH-ELP, and TO-ELP. TLP was obtained from observer-blinded manually measured and validated values from 25 Scheimpflug images obtained over 1 second. Correlation coefficients, paired t-tests and Bland Altman plots were obtained.

Results:

The mean follow-up was 105 days. The mean TLP was 4.96mm (SD: 0.28). Mean AD-iOCT prediction was: 5.02mm (SD 0.32) with a correlation coefficient of r=0.88 (p<0.01). Mean 5 variable preop biometry based TO-ELP was 5.73 (SD:0.56), r=0.74 (p<0.01). Mean 3 variable TH-ELP was 5.73 (SD:0.60), r=0.59 (p<0.01). A preliminary non-linear fit was performed to create a model to help predict refractions

Conclusions:

AD-iOCT data can more accurately predict actual MV-pELP than pre-op biometry based TH-ELP or TO-ELP, which may help improve refractive outcomes in a new non-linear fit model.
