Abstract Details

Purpose:

To report 2 cases of intraocular lens (IOL) opacification after pars plana vitrectomy (PPV) involving intravitreal injection of gas.

Setting:

Department of Ophthalmology, Kaplan Medical Center, Rehovot, Israel.

Methods:

Two cases of hydrophilic intraocular lens (IOL) opacification confined to the pupillary area are reported. One patient underwent PPV with intravitreal gas injection for macular hole and the other one for retinal detachment repair.

Results:

Both cases were associated with gas injection into the vitreal cavity that produced a central area of IOL opacification over the pupillary zone, confined to the anterior surface of the IOL. The opacification was noticed several months after PPV. In both patients, the opacified IOL was explanted and replaced by a hydrophobic IOL. The lenses underwent microscopic evaluation.

Conclusions:

IOL opacification can occur after PPV and intravitreal gas injection. The opacification can involve the anterior IOL surface and require IOL explantation. A hydrophobic IOL may be preferred when a simultaneous phacoemulsification and vitrectomy with intravitreal gas is performed.
