Abstract Details

Purpose:

To estimate a hypotensive effect and the level of safety of this operation.

Setting:

Multidisciplinary Science and Technology Complex Eye Microsurgery named after Academician S. N. Fyodorov, Federal State Institution, Novosibirsk Branch, Russia. Institute of Laser Physics Siberian Branch Russian Academy of Sciences, Novosibirsk, Russia.

Methods:

Non-penetrating glaucoma surgery by using of 308 nm excimer laser ab externo was performed in 29 patients (30 eyes) with primary open angle glaucoma (II, III stage). The flap separation and the elimination of the deep flap of sclera carried out with laser energy, delivered by flexible fiber light guide with easy-to-use handle. Than excimer laser influenced on the trabecule-descemet membrane making it thinner. The observation period was 2,5 years. Drainage paths were estimated with a help of UBM. The stability of the visual acuity, visual field were analyzed. The optic nerve state was estimated with optical coherent tomography.

Results:

A non-reactive post-op period was noted in all cases. The stable hypotensive effect was in 74% (20 cases). UBM control showed that created outflow tract is fully functioning in most cases. Additional laser descemeto-gonio puncture was required in 15 % (4 cases). Additional hypotensive drop therapy was necessary in 11% (3 cases). Visual acuity, visual field, optic nerve state demonstrated stability for all observation period.

Conclusions:

The introduced method of the surgical glaucoma treatment is simple, safe and comfortable for the surgeon. This method is highly effective. The method is appropriate for the further usage in clinical practice.
