Pre-clinical metrics to predict through focus visual performance in pseudophakic patients

Abstract Details

Purpose:

To estimate the correlation between the clinical visual acuity (VA) measured in patients implanted with different IOL models and the predicted VA provided by pre-clinical metrics based on optical-bench testing.

Setting:

AMO Groningen, The Netherlands

Methods:

The through focus modulation transfer function (MTF), phase transfer function (PTF) and US Air Force (USAF) pictures for a 3mm pupil in white light were measured using an optical bench for two monofocal IOLs, three multifocal IOLs (MFIOLs) with different add powers (2.75, 3.25 and 4D) and an extended range of vision IOL. Five preclinical metrics were calculated for each defocus position from -3D to 0D in 0.5D steps and compared to the binocular high contrast visual acuity measured at the same vergence in patients implanted bilaterally with the same IOL model, using the ETDRS chart with best distance correction.

Results:

The pre-clinical metrics were statistically significantly correlated with the clinical VA. The absolute difference between the clinical VA and the prediction of the pre-clinical metrics between 0D and –3.0D was lower than 0.1LogMAR for most of the defocus positions and most of the pre clinical metrics. The evaluation of the average visual performance calculated by the area under the throughfocus VA showed that monofocal and MFIOL with the highest add power had lower intermediate performance than the MFIOLs with lower add powers, while the extended range of vision IOL had the best visual performance in the intermediate range.

Conclusions:

Pre-clinical metrics based on optical bench testing were statistically significantly correlated with the clinical VA and therefore, may be a useful tool for assessment of the through focus binocular VA provided by an IOL from pre clinical data. Additionally, the area under the defocus curve provides a single value to estimate the average visual acuity in the intermediate range and can be used to rank the intermediate visual performance of different IOL models.
