Abstract Details

Purpose:

We evaluated the contrast sensitivity after implantation of diffractive multifocal intraocular lens (IOL) to assess the impact of previous myopic laser in situ keratomileusis (LASIK).

Setting:

Tokyo Dental College Suidobashi Hospital, Tokyo, Japan

Methods:

This retrospective case series reviewed 17 eyes of 17 patients who received diffractive multifocal IOLs after myopic LASIK at a single institution. Contrast sensitivity was measured and the area under log contrast sensitivity (AULCSF) was calculated one month postoperatively. Degradation in contrast sensitivity after multifocal IOL implantation was assessed by keratometry, corneal eccentricity, ocular axial length, central corneal thickness, ocular higher-order aberration, corrected distance and distance-corrected near visual acuities, manifest refraction, and patient age using multivariable regression analysis.

Results:

There were significant correlations except for contrast sensitivity at 18 cycles per degree (cpd) (P<0.042). AULCSF and contrast sensitivity at 3 and 12 cpd were significantly decreased in eyes with a thinner central corneal thickness (P<0.043). Contrast sensitivity at 3 and 12 cpd significantly decreased for distance-corrected near visual acuity (P=0.025) and eccentricity (P= 0.008).

Conclusions:

Factors due to the previous LASIK influenced degradation of contrast sensitivity after multifocal IOL implantation in post-LASIK eyes. Multifocal implantation could exacerbate the poor contrast sensitivity, so it is crucial to understand the risk for the patient, and careful patient selection is advised in the use of multifocal IOL after LASIK.
