Abstract Details

Purpose:

To evaluate tear film lipid layer thickness (LLT) after cataract surgery and compare the result with other tests evaluating dry eye syndrome and meibomian gland dysfunction (MGD).

Setting:

Setting : Prospective observational study Venue : Severance Hospital, Seoul, South Korea

Methods:

We evalulated 20 eyes of 12 patients who underwent cataract surgery. Patients were evaluated preoperatively and postoperatively at one month after the surgery. LLT was measured using interferometer (Lipiview; TearScience Inc., Morrisville, NC). Dry eye syndrome was evaluated with tear-film breakup time (TBUT), Schirmer test, ocular surface staining and Ocular Surface Disease Index (OSDI) score. MGD was measured with lid margin abnormalities, meibum expressibility and meibum quality.

Results:

LLT was significantly thinned from preoperative value of 83.85±23.47 interferometry color units (ICU) to 75.15±17.40 ICU postoperatively. (p=0.027) TBUT was significantly decreased from 5.91±1.81 seconds to 3.82±1.17mm. (p=0.016) Schirmer test and ocular surface staining showed no significant change. OSDI score was significantly worsened from 23.54±16.59 to 39.43±18.59. (p=0.046) Three parameters for MGD were worsened after the surgery and change of meibum expressibility showed significant difference. (p=0.038) LLT was negatively correlated with meibum expressibility and meibum quality. (r=-0.442, p=0.040; r=-0.473, p=0.026)

Conclusions:

Tear film lipid layer thickness measured by interferometer was significantly thinned after cataract surgery. Aggravation of patients' clinical signs and symptoms of dry eye syndrome after cataract surgery might be related to thinning of tear film lipid layer thickness.
