Customized toric IOL for correction of high astigmatism post deep anterior lamellar keratoplasty:a preliminary study

Abstract Details

Purpose:

To evaluate the efficacy of customized toric IOL for the correction of high astigmatism in a population of patients previously operated by deep anterior lamellar keratoplasty (DALK) and affected by cataract.

Setting:

IRCCS-Bietti Foundation, Rome, Italy and San Giovanni-Addolorata Hospital, Rome, Italy.

Methods:

in this perspective clinical study were enrolled 10 patients previously operated by DALK and affected by cataract. Preoperative astigmatism was measured with corneal topography (Syrius, CSO) , with automated keratometry (IOL master, Zeiss) and manifest refraction. Astigmatism was stable since 1 year from the removal of the corneal suture. Facoemulsification with IOL was performed in all the patients. All patients undergone a toric customized one piece acrilic idrophilic IOL (Soleko Fill 611T) . Un-Corrected Visual Acuity (UCVA), Best Corrected Visual Acuity (BCVA) , manifest astigmatism and corneal astigmatism were evaluated pre and postoperative until 12 months .

Results:

All the surgeries were performed without complications. UDVA (logMAR) improved from 1.20±0.65 preoperatively to 0.35 ±0.21 postoperatively (P-value.0.005). BCVA improved from 0.44±0.35 to 0.25±0.28 (P-value.0.01). Corneal astigmatism preoperatively was 5.75±4.50 diopters (D). Post-operative refractive cylinder was 2.35±1.75D. Analysis of astigmatic Cartesian x and y coordinates found a significant reduction postoperatively compared to preoperatively respectively. Mean endothelial cell loss was 12.3%.

Conclusions:

The IOL were effective in improving UCVA, BCVA and for the correction of manifest astigmatism post DALK. Implantation of a customized toric IOL represent an effecting modality in the treatment of cataract patients presenting high corneal astigmatism due to DALK.
