Abstract Details

Purpose:

We aim to present the therapeutic method that we usually choose when dealing with cases without capsular support.

Setting:

Clinical Emergency Hospital of Ophthalmology, Bucharest, Romania.

Methods:

We present a surgical technique used for a patient with ocular trauma in childhood. As ocular examination shows aphakia and lack of capsular support, we decided for a sutureless scleral fixation of a three-piece IOL. Our method consists in externalization of the first haptic using a 26 G needle, while the other haptic is pulled out through a scleral tunnel with the help of a 25G retinal forceps. Thus avoid the more difficult technique Yamane imagined, where for the scleral wall fixation you need a 27 G needle passed through the cilliary sulcus using the double needle technique.

Results:

Although postoperative results are similar using other methods of anchoring the artificial lens to the sclera, this technique proved to be easier because of the faster externalization and particular scleral wall fixation of the haptics. The result was successful, with no postoperative astigmatism and an uncorrected vision of 20/20 which was stable in time at 3 and 6 months.

Conclusions:

We consider this method to be a good approach for dealing with cases with lack of capsular support. It can be considered an alternative to iris claw lens or scleral suture of a posterior chamber intraocular lens.
