1.5 mm micro incision lenticule extraction surgery with minimal invasion: a possibility clinical analysis

Abstract Details

Purpose:

To report the possibility, safety and efficacy after 1.5 mm or less micro incision lenticule extraction (1.5 mm MILE) to correct myopia or myopic astigmatism.

Setting:

Tianjin Eye Hospital & Eye Institute, Tianjin Key Laboratory of Ophthalmology and Visual Science, Tianjin Medical University, Tianjin, China.

Methods:

Preliminary study. 1.5 mm MILE procedure was performed with micro scalpel in one eye and small incision lenticule extraction surgery (SMILE) with a 3-5 mm incision in the other eye of the same patient by randomized assignment. The feasibility, safety, and efficacy and high order aberration were assessed before and at 1 month after surgery.

Results:

All the procedures were well performed with no complications during the surgery. Log MAR uncorrected and corrected distance visual acuity were -0.16±0.1, -0.18±0.06 in 1.5 mm MILE group and -0.15±0.1, -0.18±0.06 in SMILE group at 1 month postoperatively. The efficacy index was 1.2 and the safety index was 1.5 in both groups at 1 month after surgery. The high-order-aberration RMS (HOA RMS ), the HOA Effect Blur index, vertical coma (Z7) and spherical aberration (Z12) were significantly increase at 1 month postoperatively compared to preoperative in both two groups (P<0.05 for all). However, there were no significant differences of these 4 parameters between 1.5 mm MILE group and SMILE group (P<0.05 for all).

Conclusions:

1.5 mm MILE is feasible, safe, and effective procedure for myopia and myopic astigmatism correction. However, it needs special technique. And the further investigations need to be performed for those advantages such as astigmatism correction and biomechanical properties or disadvantages.
