Complications of small-incision lenticule extraction procedures with liquid infiltration and center positioning in more than 4000 myopic eyes

Abstract Details

Purpose:

To evaluate the safety and complications of small-incision lenticule extraction (SMILE) with liquid infiltration and center positioning.

Setting:

Ophthalmology of Daping Hospital, Third Military Medical University, Chongqing city,PR, China

Methods:

A total of 2153 patients (4224 eyes) with myopia and myopic astigmatism underwent SMILE with liquid infiltration and center positioning in both eyes after preoperative examinations and followed for at least 3 months who were treated between January 2013 and September 2014 at Daping Hospital, Chongqing, China..Main outcome measures are surgical complications and corrected distance visual acuity (CDVA). Preoperative spherical equivalent refraction was −1.25 to −10.00 diopters (D), myopic astigmatism was 0.00 to −5.25 D, corneal curvature was 38.50 to 47.25 D and corneal diameter was 10.00 to 12.50mm.

Results:

By 3 months, all eyes had unchanged or improved CDVA; loss of 1 or more lines was not observed in any eyes. Intraoperative complications included dark spots (258 eyes, 6.11%), opaque bubble layer (253 eyes, 5.99%), laceration at the incision (169 eyes, 4.00%), suction loss (16 eyes, 0.38%); no lenticule rupture or remnant was noticed during operation. Postoperative complications comprised fiber in interface (17 eyes, 0.40%) monocular ghost images (15 eyes, 0.36%), haze of any grades, fluid in interface, epithelial islands at the incision, or infectious keratitis was not observed. Overall, patients were satisfied with visual outcomes.

Conclusions:

SMILE with liquid infiltration and center positioning performed well in correction of myopia and myopic astigmatism throughout 3-month follow-up, but it still needs further evaluation for long term.
