Purpose:
To evaluate the causes of explantation of Phakic IOLs,the time elapsed between implantation and explantation and the management of refractive error after explantation.
Setting:
Hospital ArrĂ¡bida ,Porto,Portugal
Methods:
28 eyes implanted with phakic IOLs were explanted. 11 eyes were implanted with angle-supported phakic IOLs (ZB, ZB5M; Nuvita and Acrysof Cachet), 11 eyes with iris-supported phakic IOLs (Artisan,Artiflex) and 6 eyes with posterior chamber phakic IOLs (ICL,PRL). 26 eyes were myopic and only 2 hyperopic. The causes of explantation were evaluated as well as the time elapsed between implantation and explantation . After explantation th refractive error was managed in different ways according to the patient clinical situation
Results:
The main causes for explantation of Phakic IOLs were cataract (14 eyes),followed by endothelial cell loss (5 eyes). 2 eyes had cataract and endothelial cell loss combined. Other causes included pupil ovalization and iris atrophy (4 eyes) subluxation of the PIOL (2 eyes) and intraocular inflammation (1 eye). The time elapsed between implantation and explantation was 11.9 years (range from 8 months to 21 years). After explantation phaco was performed in 22 eyes (78%) with implantation of an IOL in the capsular bag. In 5 eyes the PIOL was exchanged by a different type of PIOL.
Conclusions:
Most phakic IOLs are to be explanted one day.in our experience the mean time between implantation and explantation is 12 years. Cataract is the most common cause for explantation and phaco with implantation of an IOL in the bag is the preferred way of managing the refractive error.
