Abstract Details
Purpose:
To evaluate the load resistance of different surgical wound configuration (Mushroom, Anvil and conventional trephination) in penetrating keratoplasty (PK) by designing a 2D finite element biomechanical model of the cornea.
Setting:
U.O. Oculistica, “Nuovo Ospedale Santo Stefano”, Prato, Italy
Methods:
A mathematical model of the human cornea was developed. Multiple simulations were designed, describing different geometric configurations for PK: the internal pressure was then raised up until we observed wound misalignment and ultimate wound prolapse.
Results:
Better wound resistance was found in conjunction with all profiled laser trephinations in comparison with the conventional one. The anvil profile was more resistant to internal pressure than mushroom pattern.
Conclusions:
A few suture apposition and early suture removal in femtosecond laser-assisted anvil profile are possible because of better wound mechanical load resistance. This can contribute to faster visual recovery, helping to prevent wound rupture after trauma. However, further studies are required to identify the optimum profile including the various technical parameters.
