Abstract Details
Purpose:
To evaluate outcomes following implantation of a supplementary toric intraocular lens (TIOL) in two Chinese patients with high residual postoperative astigmatism.
Setting:
Queen Mary Hospital, Pokfulam, Hong Kong
Methods:
Following a discussion of the various options to neutralize corneal astigmatism, two patients with high residual postoperative astigmatism were implanted with the Sulcoflex toric 653T IOL (Rayner Intraocular Lenses Limited, Hove, UK). Prior to implantation of the supplementary lens , Patient A's subjective refraction (OD) was plano/-8.00 x 110 degrees, with an uncorrected distance visual acuity of 6/18. In Patient B, manifest refraction of the right eye showed a best-corrected visual acuity of -5.50 D/-6.00 D x 95 (6/7.5), and a spherical equivalent (SE) of -8.50 D. The left eye was -0.50 D/-0.50 D x 180 (6/6).
Results:
Patient A: At one week postoperative, refraction was +0.50 D/ -2.50 D x 75 (6/12), with an SE of -0.75 D. Examination of the supplementary IOL showed that the axis remained aligned at 20 degrees with good stability within the sulcus, even at Month 8, with a refraction of +0.50 D/ -2.50 D x 80 (6/9.5). Patient B: At 6 months postoperative, manifest refraction showed a spherical error of -2.00 D/-1.50 D x80 (VA: 6/4.5, MRSE -2.75 D) with minimal rotation (6 degrees) of the supplementary lens. In both cases, the postoperative course was uneventful.
Conclusions:
A sulcus-placed supplementary toric IOL is a safe and effective alternative for patients with significant postoperative astigmatism.
