Purpose:
To evaluate the accuracy of the toric intraocular lens (IOL) calculation made by an Eom's IOL add power calculator based on the effective lens position with both anterior and posterior corneal radii.
Setting:

Methods:
Twenty cataract patients (20 eyes) implanted with a Tecnis 1-piece toric IOL were enrolled. The predicted residual astigmatism error was defined as the vectorial difference between the predicted residual astigmatism and the postoperatively measured residual astigmatism. The predicted residual astigmatism error was calculated using two methods. One only considered the anterior corneal radius, and the other incorporated the effective lens position along with both anterior and posterior corneal radii. The toric IOL cylinder power and its axis correcting the total corneal astigmatism predicted by the IOL add power calculator were compared with ray-tracing simulation results.
Results:
The median magnitude of the predicted residual astigmatism error by the IOL add power calculator (0.29 CD) was significantly smaller than that by the anterior corneal radius alone (0.50 CD) (P < 0.001). The toric IOL cylinder power and its axis, correcting the total corneal astigmatism predicted by the IOL add power calculator, was strongly correlated to that predicted by ray-tracing simulation (R2=1.000, P<0.001 and R2=1.000, P<0.001, respectively).
Conclusions:
The Eom's IOL add power calculator is simple but accurate method to predict the toric IOL cylinder power and its axis based on the effective lens position with both anterior and posterior corneal radii.
