Abstract Details

Purpose:

To evaluate long-term clinical outcomes in patients after combined DMEK-S and cataract surgery.

Setting:

Eye Department of Teaching Hospital Kralovske Vinohrady and 3rd Medical Faculty, Prague, Czech Republic

Methods:

41 eyes of 31 patients were enrolled in a nonrandomized retrospective single center study. In all cases we preceded combined DMEK-S and cataract surgery in one single procedure. The follow up time was 29 months (SD±16). The main indication was endothelial corneal dystrophy and presence of cataract. Following parameters were evaluated: UCVA, BCVA, spherical equivalent and endothelial cell count.

Results:

Mean UCVA was 0,67 (± 0,23 SD), mean BCVA was 0,9 (± 0,15 SD). Mean postoperative spherical equivalent was 0,44 (± 1,1 SD). Endothelial cell density was 1187 (± 535 SD). Eleven eyes (28%) underwent rebubbling in early postoperative period. In four patients it was necessary to start antiglaucomatous treatment during postoperative period. Three patients developed Urrets-Zavalia syndrome in very early postoperative period, two patient had to underwent reDMEK because of graft failure. There was no graft rejection.

Conclusions:

DMEK-S combined with cataract surgery proved to be safe and effective method in patients with endothelial dystrophy and cataract. It shows stabile and very good long-term results of visual acuity and satisfactory endothelial cell density of the graft.
