Abstract Details

Purpose:

The aim of this study was to evaluate the feasibility and clinical outcomes of Descemet Membrane Endothelial Keratoplasty (DMEK) in complex cases, i.e. with simultaneous lens plane reconstruction in the same session.

Setting:

Non-randomized, prospective study, at a tertiary referral center. Department of Ophthalmology, Medical faculty of the Johannes Kepler University, Linz, Austria.

Methods:

Out of a total of 150 eyes that underwent DMEK surgery eight eyes underwent DMEK with simultaneous lens plane reconstruction an were enrolled in our prospective study. In 4 cases an anterior chamber IOL had to be removed first, followed by implantation of a retropupillary iris-claw fixated IOL and subsequent DMEK surgery in the same session. Four eyes were aphakic at time of surgery. In two cases a sclera- fixated IOL and in two further cases a sulcus fixated IOL was implanted prior to DMEK surgery in the same session.

Results:

All surgeries were uneventful and the DMEK graft attached properly. All eyes showed a clear cornea. The central pachymetry decreased from preoperatively 758μm (±133 μm) to 517 μm (±48 μm) 6 months postoperatively.

Conclusions:

DMEK is suitable for complex cases, i.e. combined with lens plane reconstruction in the same session.
