Recurrence rates and visual outcomes in anterior corneal dystrophies following anterior lamellar keratoplasty

Abstract Details

Purpose:

To evaluate long term visual outcomes, graft survival and recurrence rates in patients undergoing anterior lamellar keratoplasty (ALK) for anterior corneal dystrophies.

Setting:

Tertiary eye care centre in Asia - Singapore National Eye Centre, Singapore.

Methods:

Prospective cohort study of patients who underwent either penetrating keratoplasty (PK) or ALK surgery for anterior corneal dystrophies from 1997 to 2013. Data was extracted from the ongoing Singapore Corneal Transplant Study database. Chi-square and Fisher's Exact test was conducted to compare between groups. Survival rates of corneal transplant were determined using Kaplan-Meier method. Mantel-Cox Log Rank test was used to compare between 2 survival curves.

Results:

Forty three eyes in PK (mean age - 57.06 years) and 63 eyes (mean age 50.16 years) in ALK were included in this study. the 3 most common preoperative diagnoses were Lattice dystrophy (PK - 19, ALK - 24), granular dystrophy (PK-7, ALK - 18) and Macular dystrophy (PK-8, ALK-8). The 1, 5 and 10 year graft survival was 90.4%, 81.8% and 71.6% for PK and 98.3% at all time points for ALK respectively (p=0.013). Epithelial problems, glaucoma and endothelial rejection was significantly higher in the PK group. Recurrence rates were similar between the 2 groups. The best-corrected visual acuity was similar between the 2 groups at all time points.

Conclusions:

ALK eliminates the risk of endothelial rejection and provides better graft survival compared to PK. Recurrence rates and visual outcomes are similar between the 2 groups.
