Abstract Details

Purpose:

To describe a new procedure to improve the surgeon´s experience and the surgical maneuvers performing anterior lamellar keratoplasty with ZEISS rescan 700 system, which combines the surgical images with OCT.

Setting:

Vall d´Hebron Hospitals,Barcelona, Spain

Methods:

We present our experience performing DALK with or without the big bubble technique aided by OCT. This new technology provides a high quality image during the surgery (with Lumera Surgical Microscope), that can be improved with the OCT image at any moment of the surgical procedure.

Results:

OCT images during the surgery provide additional information to the cornea surgeon, guide the anterior lamella dissection, inform about the remaining stroma and the Dua/ Descemet´s membrane integrity and above all, tells us about graft adherence and what is happening at the interface and the anterior chamber during the surgery. A better understanding of the adherence of the graft improves the surgical results. The OCT images give sense to some of the tips we are using in our procedures.

Conclusions:

OCT guided anterior lamellar keratoplasty is a great leap forward for the cornea surgeon, for the understanding of surgical maneuvers and graft adhesion during the procedure. It is and advantage that will change the way we plan and perform anterior lamellar surgery, an explanation to some questions about postoperative results and complications, and an accessory technology that will become necessary in the future.
