Purpose:
To evaluate the differences in visual performance following either corneal refractive surgery or phakic intraocular lens implantation.
Setting:
IOA Madrid Innova Ocular, Madrid, Spain
Methods:
100 eyes from 100 patients undergoing myopia and/or myopic astigmatism refractive surgery were included in the study and were split into 4 groups (25 eyes each): ICL, ReLEx smile, Femto-LASIK, and PRK. Intraocular scattering (OSI), modulation transfer function (MTF) and Strehl ratio (SR) were measured using OQAS (Visiometric, Terrasa). Moreover, both pre- and post-operative VA were assessed, resulting in the corresponding Safety Index (SI) and Efficacy Index (EI) values.
Results:
The best outcomes in terms of SI and EI were achieved for PRK or ICL implantation. Regarding visual performance, the best OSI results were again for PRK and ICL, with statistically significant differences (p < 0.05) between these groups and the Femto-LASIK and ReLEx smile ones. As for MTF, it was PRK that yielded the best outcomes. The differences were statistically significant relative to ReLEx smile and Femto-Lasik, but not relative to ICL implantation. For SR no differences were found among the 4 techniques. Finally, the largest improvement in terms of VA (post-vs-pre-operative values) was observed in the ICL group.
Conclusions:
Refractive surgical techniques, though they most often lead to spectacle independence, they may result in visual performance deterioration, which is more pronounced in those surgical approaches where the corneal treatment is applied in deeper corneal layers.
