Small incision lenticule extraction (SMILE): results from treatment of more than 1000 eyes with myopic astigmatism

Abstract Details

Purpose:

The aim of this study was to evaluate refractive predictability and visual acuity up to 3 months after SMILE treatment for all degrees of myopia.

Setting:

Department of Ophthalmology, Odense University Hospital, Odense, Denmark.

Methods:

Retrospective study of results after SMILE treatments. All treatments were performed from 2011 to 2014. Inclusion criteria:Corrected Distance Visual Acuity (CDVA) ≤ 0.10 (logMAR) before surgery, and no other ocular conditions than myopia up to -10 D with or without astigmatism of maximum 3 D. Exclusion criteria: Eyes having undergone re-treatment. All treatments were performed with a VisuMax® femtosecond laser (Carl Zeiss-Meditec, Jena, Germany). Lenticule diameter ranged from 6.00 to 6.60 mm. Maximum attempted spherical correction was -10.00 D. Clinical examinations were performed pre-operatively and at 1 day, 1 week and 3 months post-operatively.

Results:

Of 1150 treated, 983 eyes were included. Pre-operative spherical equivalent (SE) refraction averaged -6.77±1.74 D. Emmetropia was target refraction in 362 eyes. In these eyes, 83% had an UDVA ≤ 0.10 (logMAR) after 3 months. Of all eyes, 88% were within ±0.5 D of intended refraction, whilst 98% were within ±1.0 D at 3 months after surgery. The mean difference between attempted and achieved SE refraction was -0.08±0.71 D. Concerning astigmatism, 83% had ≤ 0.5 D after 3 months, compared to 53% before surgery (P=0.02). After 3 months, 13 eyes (1.3%) had lost 2 lines or more of CDVA.

Conclusions:

SMILE seemed predictable and safe for treatment of all degrees of myopia and myopic astigmatism up to 3 months after surgery.
