Abstract Details
Purpose:
To evaluate the changes in anterior chamber depth (cornea-iris distance, iris-lens distance) and pupil size after cataract surgery and determine whether there is an association between the changes in anterior chamber depth and pupil size.
Setting:
Prospective, nonrandomized, consecutive case study
Methods:
A prospective study was conducted on 80 eyes from 62 patients (29 men and 33 women with a mean age of 68±6.7 years) who underwent uncomplicated cataract surgery between October 2013 and January 2015. The distance between central corneal posterior surface to iris margin plane (CID) were measured preoperatively and 1 month postoperatively using a Pentacam under the same illuminance (10 lux). The distance between iris margin plane to central lens surface (ILD) and pupil size were measured using a Fourier-domain optical coherence tomography(OCT).
Results:
CID were significantly increased from 2217±491 to 3160±312µm following cataract surgery (P<0.05, Paired T-test). ILD were significantly increased from 97±57 to 920±108µm following the surgery (P<0.05). Pupil size were significantly decreased from 5.18±0.90 to 4.55±0.67mm following the surgery (P<0.05). When comparing an association between the changes of CID and pupil size, ILD and pupil size, these changes were not statistically significant (P>0.05, Pearson correlation coefficient).
Conclusions:
Anterior chamber depth (ACD) were increased and pupil size were decreased significantly after cataract surgery. As IOL is smaller and thinner than crystallin lens, IOL does not push the iris forward. This changes ACD get deeper and pupil size get smaller. There was no significant association between the changes in ACD and pupil size. As IOL does not contact with iris, the power of forward movement to iris is no longer acts, there are no more change of pupil size according to ILD. Changes in ACD of postoperative pseudophakic state would not influence pupil size.
