Abstract Details
Purpose:
To measure Higher-order aberration (HOA) in old age group and nuclear cataract and investigate the change of spherical aberration (SA) with aging or progressing nucleosclerosis.
Setting:
Dept of Ophthlamology, Wonju Christian Severance Hospital ,Korea.
Methods:
One hundred two eyes of old age group subjects (>50) were classified according to Lens opacities classification system III (LOCS III) and examined with Hartmann-Shack (HS) aberrometer for 6 mm diameter. Sixty-seven eyes with nuclear cataract (>N4; 21, N3; 23, N2; 23) were compared with 35 eyes of control group with relatively clear nucleus (< N2). To investigate lenticular aberration, HOAs were examined in 27 eyes with nuclear cataract having an aberration-neutral intraocular lens (920H, Rayner, UK) inserted following phacoemulsification, and changes of aberrations after surgery were investigated. The relationship between lens opacity grade and ocular aberrations were investigated.
Results:
The SA was significantly smaller in 67 eyes with nuclear cataract compared with 35 eyes with relatively clear lens (0.13 vs. 0.25um, P<0.001). TF6, Coma7, and SA showed significant differences in group comparison, and SA showed significant negative association with nuclear opacity grading after excluding age effect. The predominant change in HOAs after phacoemulsification was shown as the increase of SA, and 85 % of lenticular SA of nuclear cataract presumed to be negative value. The ocular and lenticular SA was negatively correlated with severity of nuclear opacity.
Conclusions:
The ocular spherical aberration increases with age, but decreases with progression of nuclear cataract due to the negative correlation of lenticular spherical aberration and severity of nuclear opacity
