Abstract Details

Purpose:

To study the various causes and final outcome of patients with peripheral ulcerative keratitis.

Setting:

This was a retrospective non randomized study done in Government medical college and hospital, Chandigarh which is a tertiary care hospital in North India.

Methods:

This study was conducted on 33 eyes of 27 patients with PUK of varied causes who reported to us in the cornea clinic between January 2007 to July 2010. The intervention done was routine history, visual assessment , slit lamp examination to assess ulcer and detail systemic examination. Routine tests like complete blood counts, ESR, urinalysis was done and special tests like LE cell, ANA factor, RF and chest X ray was done. Treatment include medical and various types of surgical interventions. Minimal period of 3 months was done.

Results:

33 eyes of 27 patients were studied. It include 22 (81%) males and 5 (19%) females. PUK was unilateral in 21(78%) and bilateral in 6 (22%) patients. Most common cause of PUK in our study was found to be infective (54.5%), Moorens ulcer (6%), Tuberculosis (6%), connective tissue disease (6%) and others (27.6%). Medical management include topical antibiotics, artificial tear substitute and cyclosporine. Non responsive patients were managed with autologus serum, systemic steroids, cyclophosphamide and lastly by surgical management.

Conclusions:

PUK is a challenging condition with multiple and varied etiology which require multimodality treatment. It represents varied spectrum of corneal disorders: which if managed properly and timely: can give excellent visual outcome.
