Purpose:
Extended range of vision lenses can provide patients a continuous range of vision of 6/9 or better from distance to about 65 centimetres. Near visual acuity at 40 cm is about 6/12 on average and may fall short of 6/9. Micromonovision can be used to improve the near vision further and achieve the optimal uncorrected binocular visual acuity of 6/9 or better from distance to near
This study presents a functional vision evaluation of patients undergoing lens exchange surgery using Micromonovision refractive targeting with an Extended Range of Vision Intra-ocular lens.

Methods:
30 consecutive patients undergoing lens exchange surgery using customised micromonovision targeting with an extended range of vision intra-ocular lenses were enrolled. The first eye refractive target of was emmetropia. The refractive target for the second eye was between -0.25 to -0.75 Dioptres. Four to 8 weeks after second eye surgery their monocular, binocular, uncorrected and distance corrected visual acuities for distance, intermediate and near (photopic and mesopic) were measured. Objective task assessment and a patient questionnaire were also administered.
Binocular uncorrected acuity of 6/9 for distance, intermediate, near photopic and mesopic conditions was the primary outcome measure.
Results:
96% of patents achieved a binocular uncorrected visual acuity of 6/9 or better for distance, intermediate and near in photopic conditions. 52% of patients achieved a binocular visual acuity of 6/9 or better for near in mesopic conditions while 92% of patients achieved a binocular visual acuity of 6/12 for near in mesopic conditions. 98% of patients successfully performed intermediate and near tasks in photopic conditions. 50% of patients were able to successfully perform near tasks in mesopic condition. The subjective patient questionnaires were consistent with the objective task results. No significant dysphotic effects were reported.
Conclusions:
Customised micromonovision using an extended range of vision lens gives excellent functional visual acuity results. The vast majority of patients achieve a full range of spectacle free vision about half the patients needed some help to carry out fine vision tasks in mesonic conditions. Patient perception of their functional vision is excellent with no significant reports of dysphotic effects.
