Abstract Details
Purpose:
To determine the safety and efficacy of photorefractive keratectomy (PRK) and transepithelial photorefractive keratectomy (tPRK) combined with simultaneous High Energy accelerated corneal Crosslinking in patients with early keratoconus.
Setting:
Muscat Eye Laser Center, Muscat, Sultanate of Oman.
Methods:
We included 18 eyes of 9 patients with early keratoconus treated PRK (14 eyes) or Trans-PRK (4 eyes) with the Amaris 750S (Schwind) using customized profiles, optical zones between 6.3 to 7.0, and ablations between 38 to 81µm. with simultaneous High Energy accelerated corneal crosslinking (Avedro), Follow-up was performed at months 3, 6, and 1 year including; UCVA, BCVA, refraction, keratometry, placido topography (KeratronScout, Optikon), and Scheimpflug imaging (Sirius, CSO Inc.).
Results:
This study included 18 eyes (9 patients) of mean age 27.44 ± 4.3 years (range 18-32 years). At 1 year of follow up there was a significant improvement in UCVA (0.20 vs 1; p<0.000), mean manifest refractive spherical equivalent (MRSE) (-3.22 vs -0.04; p=0.000), sphere (-2.76 vs +0.04 D; p<0.000) and cylinder (-0.91 vs –0.16 D; p=0.001). Although mean BCVA improved the difference was not statistically significant (0.97 vs 1; p=0.163).
Conclusions:
PRK or Trans-PRK followed by accelerated crosslinking provides excellent visual outcomes in early keratoconus patients. The long-term stability of these results will need to be confirmed with longer follow up.
